Personal Treatment Licences

Licences are required for a number of businesses that carry out personal treatments (e.g. acupuncture, ear-piercing).

Application forms for these licences can be downloaded by clicking on the relevant link. The form can then be printed off, filled in and sent (along with fee payment if required) to the relevant officer.

All are PDF files, if you do not have Acrobat Reader it can be downloaded free of charge from Adobe website.
